Social Profile

 

The Social Profile page is used to view the patient's social, occupational, and environmental history. There are multiple sections of patient data.

COPY FORWARD FUNCTIONALITY: When a new visit is created, the Social Profile from the previous visit is automatically copied forward from the last visit. NOTE: The first time the Social Profile is entered for a patient, it must be entered on the most recently created visit, including pre-admit visits. This ensures that the copy forward functionality works as expected for future visits.

There are some instances that the patient has simultaneous visits and the Social Profile only copies to the most recent one. In that case, the Social Profile will be blank for the visit and a Copy Forward button is displayed on the page. Click this button to manually copy forward the Social Profile information.

 The sections are grouped on the left side of the page by category:

When viewing the page, you can click on the header or the data element (listed below the category title) to jump to that section. You can also perform the following tasks:

To complete/edit the social profile

  1. To edit the Social Profile, click the Edit button located at the top right of the page.
  1. Complete the information in any of the following sections: Health Risk Factors, Administrative Data, Personal, Occupational, Social, and Environment using the steps described in the sections below.
  2. Click Save and Continue while working to save your work as you go. Click Save to save your changes, exit edit mode, and return to the Social Profile page view.   

Health Risk Factors

There are 4 data elements in the Health Risk Factors section: Smoking Status, Alcohol Use, Drug Use, and Additional Questions.

I. Smoking Status

  1. In the Smoking Status section, check the applicable options in the Tobacco Use/Exposure and the Current Smoking Status lists. For most option, once an item is selected, a calendar icon is displayed next to it. Click the calendar icon to document the Start Date and Stop Date for the selected option using the Year, Day, or Month formats.
  2. The Tobacco Use / Exposure section has an option for Additional "Tobacco Use/Exposure" Sections. You can search for and add additional terms/items to add to the patient's record using the SNOMED database. Begin typing the item you'd like to add, select it, then click the plus button to add it to the record.  To remove an additional item, click the minus button located next to the item.

Pack years is calculated based on amount per day or week and the length of time (years). For more information on pack years and a calculator, see http://smokingpackyears.com/.

  1. In the Have you used smokeless tobacco in the last thirty days field, enter Yes, No, or Refused.
  2. In the Additional Information box, enter free-text to further explain the patient's Smoking Status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

II. Alcohol Use

  1. In the Alcohol Use section, complete the questions and select the applicable options.
  2. In the Additional Information box, enter free-text to further explain the patient's Alcohol Use.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

III. Drug Use

  1. In the Drug Use section, complete the questions and select the applicable options.
  2. In the Additional Information box, enter free-text to further explain the patient's Drug Use.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

IV. Additional Questions

If present, this section includes customized questions created by the facility.

  1. In the Additional Questions section, complete the questions and select the applicable options.
  2. In the Additional Questions box, enter free-text to further explain the patient's Health Risk Factors.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

Administrative Data

There are 5 data elements in the Administrative Data section: Gender Identity, Multiple Birth, Sexual Orientation, Preferred Language, and Additional Questions.

I. Gender Identity

  1. In the Gender Identity section, complete the questions and select the applicable options.
  2. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

II. Multiple Birth

  1. In the Multiple Birth section, complete the question.
  2. In the Additional Information box, enter free-text to further explain the patient's multiple birth status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

III. Sexual Orientation

  1. In the Sexual Orientation section, check the applicable option.
  2. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

IV. Preferred Language

  1. In the Preferred Language field, use the drop-down list to select the patient's preferred language.  
  2. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

V. Additional Questions

If present, this section includes customized questions created by the facility.

  1. In the Additional Questions section, complete the available questions and/or select the applicable options.
  2. In the Additional Questions box, enter free-text to further explain the patient's administrative data.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

Personal

There are 6 data elements in the Personal section: Financial Resource Strain, Education, Stress, Depression, Physical Activity, and Additional Questions.

I. Financial Resource Strain

Based on the answers selected in this section, an MU score is displayed at the top of the section.

  1. In the Financial Resource Strain section, complete the question. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's financial strain status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

II. Education

  1. In the Education section, complete the question using the drop-down list. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's education status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

III. Stress

  1. In the Stress section, complete the question. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's depression status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

IV. Depression

Based on the answers selected in this section, an MU score is displayed at the top of the section.

  1. In the Depression section, complete the question. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's depression status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

V. Physical Activity

  1. In the Physical Activity section, complete the question. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's physical activity status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

VI. Additional Questions

If present, this section includes customized questions created by the facility.

  1. In the Additional Questions section, complete the available questions and/or select the applicable options.
  2. In the Additional Questions box, enter free-text to further explain the patient's personal data.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

Occupational

There are 3 data elements in the Occupational section: Industry and Occupation Codes, U.S. Uniformed/Military Service Data, and Additional Questions.

I. Industry and Occupation Codes

  1. In the Industry and Occupation Codes section, select the applicable options and complete the Occupation and Occupation History questions using the available drop-down lists. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

II. U.S. Uniformed/Military Service Data

  1. In the U.S. Uniformed/Military Service Data section, complete the question. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

III. Additional Questions

If present, this section includes customized questions created by the facility.

  1. In the Additional Questions section, complete the available questions and/or select the applicable options.
  2. In the Additional Questions box, enter free-text to further explain the patient's personal data.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

Social

There are 4 data elements in the Social section: Social Connection/Isolation, Exposure To Violence, Religious/Cultural Beliefs, and Additional Questions.

I. Social Connection/Isolation

Based on the answers selected in this section, an MU score is displayed at the top of the section.

  1. In the Social connection/Isolation section, complete the questions. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's social status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

II. Exposure To Violence

Based on the answers selected in this section, an MU score is displayed at the top of the section.

  1. In the Exposure to Violence section, complete the questions using the available options. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's exposure to violence status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

III. Religious/Cultural Beliefs

  1. In the Religious/Cultural Beliefs section, select the applicable option and complete the question. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's cultural beliefs status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

IV. Additional Questions

If present, this section includes customized questions created by the facility.

  1. In the Additional Questions section, complete the available questions and/or select the applicable options.
  2. In the Additional Questions box, enter free-text to further explain the patient's personal data.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

Environment

There are 2 data elements in the Environment section: Home Environment and Additional Questions.

I. Home Environment

  1. In the Home Environment section, select the applicable option for the Activities of Daily Living and Housing Status questions and complete the question. Or, check Refused if the patient is unwilling to answer.
  2. In the Additional Information box, enter free-text to further explain the patient's home environment status.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

II. Additional Questions

If present, this section includes customized questions created by the facility.

  1. In the Additional Questions section, complete the available questions and/or select the applicable options.
  2. In the Additional Questions box, enter free-text to further explain the patient's personal data.
  3. If the section is completed, check the Completed box. The Completion Date/Time default to the current. Modify the date/ time as needed using the calendar and clock icons.

To document that there is no change

When reviewing the content of the Social Profile, you can mark the section as No Change. This means that there is no change in the information documented in that section of the form . To document the No Change stamp, click the No Change button located in the header for that section.

To View the change log

The Change Log details the actions taken, data entered, and edits made to the section. To view the Change Log, click the Change Log button. The log is displayed with columns for Date/Time, User, and Action.
